Checking Your Club's Vital Signs

The idea is that clubs (individuals within a club) complete this questionairre before they invite new members. The second column is to be completed by the members acting in the role of (or pretending to be) a prospective new member.

The key to this exercise is to not invite any one until the responses are all in both the first and second columns.
